As your local FQHC since 1971, we break down barriers with affordable care through our sliding fee program, and uniquely, we tackle food insecurity head-on by linking patients to SNAP and local food support. We're not just treating symptoms; we're building healthier, stronger neighborhoods from the ground up.
Since 1971, Neighborhood HealthSource, an FQHC, has delivered affordable, accessible healthcare to North and Northeast Minneapolis and northern suburbs, advancing health equity. We ensure no cost barriers with our sliding fee scale (visits from $20-$75) and accept most insurance. Our Patient Access Coordinators offer free insurance and sliding fee assistance, reducing out-of-pocket expenses. We're committed to quality care for all.
Our dedicated MNsure navigators address food insecurity, helping with SNAP applications (improving health and chronic disease outcomes), connecting patients to food shelves, and listing free local food resources. Our FoodRx program, a past Minneapolis-funded fresh food delivery pilot, is now a research focus, demonstrating fresh food's positive health impacts. We envision a community where no one faces financial barriers to comprehensive care or healthy food.

Neighborhood HealthSource (NHS) is deeply rooted in the communities we serve. Our robust Outreach team regularly conducts vital outreach on Central Avenue in Northeast Minneapolis, including at the Ecuadorian Consulate. This dedicated effort ensures we connect individuals directly with our much-needed clinic services, making quality healthcare accessible to all.
